Program Description: The Southern Nevada Health District’s Office of Chronic Disease Prevention and Health Promotion (OCDPHP) addresses prevalent and costly chronic diseases like cardiovascular disease, obesity, and diabetes. The CDC identifies four modifiable behaviors—tobacco use, insufficient physical activity, poor diet, and excessive alcohol use—as major contributors to these diseases. The OCDPHP, staffed with trained health educators and community health workers, collaborates with community partners to promote physical, social, and policy changes that reduce health disparities and facilitate healthy living. The office is dedicated to delivering creative, culturally sensitive, and evidence-based health promotion programs.

Services Offered: The OCDPHP collaborates with various organizations and community members to offer innovative, culturally sensitive, and evidence-based health promotion programs to prevent and help self-manage chronic disease. The OCDPHP team strives to enhance community health by encouraging residents to address health issues, educating and motivating people to adopt healthy lifestyles, and modifying policies, systems, and the physical environment to promote healthy behaviors. Together with community partners, the OCDPHP works to transform the community by promoting physical activity, improving access to nutritious food and drinks, and minimizing exposure to second-hand smoke.

OCDPHP develops and implements education programs throughout the year to help manage chronic disease conditions. Programs include:

  • Diabetes Self-Management, Education & Support Classes
  • Barber/ Beauty Shop Health Outreach Project to provide free blood pressure and prediabetes screenings, education, and referrals
  • Smoke-Free Housing Directory that includes smoke-free apartment units in Southern Nevada
  • Free mobile apps and online programs to support healthy eating, physical activity, and diabetes prevention
  • School and community physical activity programs
  • Pop-Up Produce Stands that accept SNAP benefits
  • English and Spanish websites, blogs, and social media sites to share credible health information and available resources
